Medical Release & Liability

I hereby grant permission for my child to participate in all Temple youth programs, activities and events and do release Congregation Shir Ha-Ma’alot and its representatives from all liability arising out of my child’s participation in such activity. 

In addition, I, the undersigned parent/guardian of the above child, do further certify that my child is physically able to participate in such activity and hereby authorize Congregation Shir Ha-Ma’alot and its authorized representatives as agents for the undersigned, to consent to any x ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care which is to be rendered under the general or specific supervision of any licensed physician (under the provisions of the California Medicine Practice Act) or the staff of a licensed hospital, whether such diagnosis, examination or treatment is rendered at the office of the said physician, or at such hospital. 

It is understood that this authorization is given in advance of any specific examination, diagnosis, treatment, or hospital care being required, and is given to provide authority and power on the part of our above named agents to give specific consent to any and all such examinations, diagnosis, treatment or hospital care which the aforementioned physician in the exercise of his/her best judgment may deem advisable. The authorization is given pursuant to the provisions of section 25.8 of the Civil Code of California.

Parent/Student Covenant

By selecting Yes below and by enrolling your child(ren) in Religious School at Congregation Shir Ha-Ma'alot, you indicate your agreement to the above terms and conditions.

Our school atmosphere is one based on mutual respect for learning and growth. In the spirit of community, and in order to achieve our goals and ensure the integrity of our program, the following is the covenant to which the student, parent and Temple agree:

Attendance Requirements:
In order to advance to the next grade level, a Religious School student must be present for a minimum of 75% of all scheduled classes and 85% of all Hebrew Chavurah classes. If your child misses a session, please notify your child’s teacher or the Temple office as make-up assignments or materials may be given in the upper grades. To progress to the next grade, a child needs to have experienced the classroom material, as each year of study builds on the foundation of previous years of learning.

Family Involvement:
Family Camp Sunday Sessions and Parent Education days are a vital component of the Shir Ha-Ma’alot educational program. Please make sure to enter the dates of your child’s family education days into your calendar now to ensure your attendance.

Standards of Behavior:

Each Religious School student agrees to:

1. Arrive on time to class.
2. Be courteous and kind to teachers and other students.
3. Treat the building and its classrooms respectfully.
4. Enter the Sanctuary with special care and respect.
5. Participate in T’fillah (prayer services) to the best of his/her ability.
6. Turn in assignments regularly and on time.
7. Participate in class activities to the best of his/her ability, without intentionally causing disruptions.
8. Never act in a way that prevents another student from learning.

The School, Teachers and Parents agree to:
1. Promise to listen and respond to all issues.
2. Assist any student who needs extra help.
3. Create an atmosphere of warmth and inclusiveness.
4. Participate as a partner in the Jewish development of the student.
